Covid Acknowledgement

Currently or within the past ten (10) days I have not experienced any symptoms associated with COVID-19.

I have not been exposed to a confirmed or suspected case of COVID-19 or have not tested positive or been diagnosed with COVID-19 in the last ten (10) days.

I knowingly and freely assume all risks (including death) elated to illness and infectious diseases, such as COVID-19.

I agree to abide by the recommended mitigation protocols in an effort to keep all participants and volunteers safe (i.e. mask wearing, social distancing, hand and table hygiene, etc.).
